Beauty consultation and consent form template
A beauty consultation and consent form records what a client wants done, whether it is suitable and that they understand and agree to it before treatment starts. This version covers the client's details, the treatment and price, an age check, health questions, a patch test, risks and aftercare, an optional time to think, consent to using health information and photos, and signatures. It is completed at the first consultation and again for each new treatment or course, with the client, treatment, answers and dates different each time.

The rules it usually needs
Written in plain English when the template is set up.
- One of several wordings
“The age confirmation says either that the client is 18 or over, or gives their age, with a parent or guardian signing the separate section.”
When the client is 17, the form states that age and the parent or guardian section is included.
- One of several wordings
“A patch test is recorded with its date, site and result, or the form says why one is not needed for this treatment.”
For a chemical peel with no patch test required, the form gives the reason instead of a result.
- Only when…
“Include the time to think section, and the prescriber details, only if the treatment needs a gap or is prescription-only.”
For anti-wrinkle injections, the earliest treatment date is set after the consultation date and the prescriber's details are shown.
- Only when…
“Include the age restrictions for England, the photo consents, and the insurance line only if they apply to this business.”
When the business takes before and after photos, the photo consent tick box appears; when it takes none, it does not.
- Once for each item
“A row for each risk discussed and each treatment-specific health question.”
Three risks were discussed, so the risks table has three rows.
Questions people ask
Do beauty clients have to fill in a consultation form?
There is no single law that requires one, but most insurers and professional bodies expect a consultation, health questions and a record of consent before treatments such as peels, lasers and injections.
Can under-18s have beauty treatments?
Some are restricted by law. In England it is an offence to give botulinum toxin to anyone under 18, or injected fillers for cosmetic reasons, with limited exceptions for doctors. Tattooing and sunbed use are also banned for under-18s. Other treatments are a matter for the business's own policy and insurer.
Is a patch test required?
Not by law, but manufacturers and insurers commonly require one before tints, some peels and similar products. The form records the test, or why it was not needed.
Is a client's health information special category data?
Yes, under the UK GDPR. The form asks for the client's explicit consent to use and keep it, says how long it is kept, and explains how to complain to the business and to the Information Commissioner's Office.
Can I use my own consultation form?
Yes. Upload your own Word or PDF version and DocuWeave turns it into a template in your wording, with the same kind of rules and the same filling from paperwork.
