Investigation meeting invitation letter template
An investigation meeting invitation letter asks an employee to attend a meeting as part of a workplace investigation. It names the investigator, says what the investigation is looking into, gives the date, time and place, names the note-taker, and asks the employee to keep the matter confidential. Employers send one to each person they need to speak to, so the employee, the matter and the meeting details change every time.

Questions people ask
Does an employee have the right to be accompanied at an investigation meeting?
There is no statutory right to be accompanied at an investigation meeting, but employers can allow it, and this letter does. The statutory right applies to disciplinary and grievance hearings.
Is an investigation meeting the same as a disciplinary hearing?
No. An investigation gathers the facts. If it finds a case to answer, a separate disciplinary hearing follows.
Why ask the employee to keep the investigation confidential?
Keeping it confidential helps the investigation to be carried out fairly. The letter says that breaching confidentiality could lead to disciplinary action.
What if the employee cannot attend?
The letter asks them to contact a named person as soon as possible so the meeting can be rearranged.
