Clinical supervision agreement template

A clinical supervision agreement sets out how a supervisor and a practitioner or trainee will work together: the format, fees, confidentiality, records and how either side can end it. Counsellors, therapists and other practitioners complete one when supervision starts, and again when the arrangement changes.

Questions people ask

What is a supervision agreement?

It is a written agreement between a supervisor and a practitioner or trainee, setting out how supervision works: the format, fees, what is confidential and what is not, and how the arrangement can be reviewed or ended.

Is supervision confidential?

The agreement says what is said is confidential between the supervisor and supervisee. It also sets limits, for example where a client or someone else may be at serious risk of harm.

Can it be used for group supervision?

Yes. There is a group format with a limit on numbers, and the agreement says group members agree to the same confidentiality.

Does it cover trainees?

Yes. An optional line records the training course and placement, and an optional section gives a 14-day right to cancel for trainees paying personally.

Who keeps the records?

The supervisor keeps brief session notes with no client-identifying details for the number of years stated. The supervisee can see the notes about them.
